What is the admission process?
Non-emergency Cases
For patients without an HMO card, or company affiliation with a corporate account with the Hospital.
- Please proceed to the Admitting Department and present your doctor’s admitting order sheet, and PhilHealth documentation (if you are a PhilHealth member).
- Fill out an admission form.
- Read through the Patient’s Rights and Responsibilities and affix your signature on the Consent, Undertaking and Waiver form.
- Depending on the diagnosis, type of procedure/treatment, and choice of room type, an initial payment shall be required upon admission.
- An admission kit will be provided to you as you enter the patient room.
- Wear your patient bracelet during your entire stay at the hospital.
For HMO cardholders or employees of companies with a corporate account with the Hospital (with and without PhilHealth)
- Please proceed to the Admitting Department to inquire about the availability of rooms.
- Upon confirmation of the room availability, you will be directed to the HMO Concierge where you must present your valid ID, HMO card/company ID and Letter of Authorization (LOA).
- At the latest, you must present your LOA within 24 hours after admission or on the first working day if you were admitted on a weekend or a holiday.
- Failure to present a LOA shall lead to the patient being responsible for her/his account with the Hospital. Hence, an initial payment shall be required upon admission.
- You will then be directed back to the Admitting Department where you will present your admitting/doctor’s order sheet and PhilHealth documentation (if available).

- An admission kit will be provided to you as you enter the patient room.
- Wear your patient bracelet during your entire stay at the hospital.
Emergency Cases
- Proceed to the Emergency Room (ER) for proper diagnosis. You will be advised by an ER doctor if you will be admitted for appropriate treatment
- If it is determined that you are to be admitted, you will be brought to the Admitting Department.
- Our Admitting personnel will advise you – or your companion – to fill out an admission form.
- Read through the Patient’s Rights and Responsibilities and affix your signature on the Consent, Undertaking and Waiver form.
- An initial payment will be required from the patient once s/he is in stable condition (as determined by the ER doctor), or 24 hours after her/his transfer to a patient room.
- An admission kit will be provided to you before you are brought to the patient room.
- Wear your patient bracelet during your entire stay at the hospital.
What are the discharge guidelines and procedure?
Discharge Process
- Once your doctor issues your discharge order, you will be informed by your nurse on when you can go home. Our billing personnel will likewise notify you on how to settle your billing obligations at our Cashier section.
- Once your hospital bill has been settled, our Cashier personnel will issue two (2) discharge clearance slips―to be signed by your nurse before you leave the room.
- The first copy is to be given to the Nurse Station while the second copy must be presented to the guard-on-duty at the main or back exit point.
- Please note that you are given a maximum of one (1) hour to vacate the room after the bill has been settled.
