What are the PhilHealth Guidelines?
For PhilHealth Members & Dependents
To ensure the swift processing of PhilHealth benefits, please accomplish the following forms prior to discharge:
- Updated Member Data Record
- Claim Signature Form (CSF)
- Claim Form 2 (CF2)
- Claim Form 3 (CF3) – for maternity cases.
If you are unable to submit the said forms before being discharged, the Hospital will not be able to deduct your PhilHealth benefits from your hospital bill.
